Wandering Risk

You’ve heard of the tragic outcomes during a patient wandering event. At Alzheimer’s Community Care, we’ve only had happy outcomes.

We provide individualized solutions and education to ensure the safety and security of identified patients with Alzheimer’s disease or a related neurocognitive disorder. Each proven safety tool is provided at little or no cost.

We have a long-standing partnership with Law Enforcement in assisting with wandering events within our three service territories of Palm Beach, Martin, and St Lucie counties.

 

Safety tools include:

  • Electronic locator devices like radio frequency bracelets and GPS systems
  • Door alert systems
  • Medical ID Bracelets
  • Scent Preservation Kits

 

To participate in this service, the patient must meet the following criteria:

  • Is at risk of wandering
  • Have a diagnosis of probable Alzheimer’s disease or related neurocognitive disorder
  • Complete an evaluation with an Alzheimer’s Community Care Family Navigator
